Activity Description

The goal of this activity is to provide pharmacists and pharmacy technicians an overview of medication errors including best practices for prevention, the importance of error reporting, and promoting a “Just Culture” environment to enhance medication and patient safety.  The program will comprehensively review primary types of medication errors, including when and where they are likely to occur, risk factors associated with medication errors and will explore medications or practices likely to be implicated in medication errors. Audience members will gain improved knowledge and understanding of the role they play in preventing medication errors and will feel empowered to promote a safe culture in the workplace.

Learning Objectives

  1. Define a medication error.
  2. Discuss the primary types and impact of medication errors.
  3. Explain the Root Cause Analysis (RCA) process and situations that can be resolved using RCA.
  4. Review Institute for Safe Medication Practices (ISMP) best practices to prevent medication errors.
  5. Identify factors that contribute to medication errors and possible barriers to reporting errors.
  6. Describe the role of a pharmacist in preventing and reporting medication errors.
