ORAL ONCOLYTIC WELCOME LETTER

This template will introduce the patient to the process of accessing and taking oral medication for cancer treatment, and key definitions of related terms.  This form can be edited to meet the needs of your department.

ORAL ONCOLYTIC NEW START CHECKLIST

This new start checklist provides a step-by-step list of important actions to be taken when oral cancer therapy is initially prescribed.

PLAN OF CARE TREATMENT GUIDE

This template includes key elements of drug specific instructions for the patient.  This form intended to be provided to patient at initial formal education session.  This document follows the guidelines of ASCO and NCODA, and can be edited to work with any type of treatment regimen.

ORAL THERAPY PATIENT FOLLOW-UP TEMPLATE (PAGE 1)

This template contains key documentation criteria for follow-up assessments of patients taking oral cancer therapy. It may be downloaded, edited, and used as a resource for developing a documentation flow sheet for oral adherence and toxicity evaluations into your practice’s EHR system.

ORAL THERAPY PATIENT FOLLOW-UP TEMPLATE (PAGE 2)

This template provides CTACE grading scale for the most common adverse events of oral cancer therapy.

FIRST FILL TRACKING FORM

This template is to assist with tracking the prescription from patients from the initial order to when the medication is dispensed.

AFTER THE FIRST FILL TRACKING FORM

This template is to assist with tracking patients throughout the spectrum of their treatment, beginning on the day they initiate oral therapy.

NEW-START ADHERENCE BARRIERS ASSESSMENT DOCUMENTATION TEMPLATE

This template is to assess patients’ potiential barriers to adherence BEFORE they begin their oral anticancer therapy.

Elizabeth Bettencourt

About Elizabeth

Elizabeth has worked in oncology for over 30 years, the last 13 years as an Oral Oncolytic Nurse Navigator at Palo Alto Medical Foundation/Sutter Health, where she developed a robust oral oncolytic program that has been expanded to other Sutter Health sites. The oral oncolytic program incorporates prescription processing and drug acquisition, collaboration with in-house and off-site pharmacists, patient and staff education, and continued adherence/toxicity monitoring for all patients during their course of treatment.

Dallas Lawry

About Dallas

Dr. Lawry has been an oncology nurse within the University of California Health System for her entire career (2014). She started at UCLA, working as an inpatient oncology certified nurse, and currently works at UC San Diego as an outpatient oncology nurse practitioner. Her passion within oncology is palliative care and end of life. She has served on the NCODA Executive Council since 2021, where she advocates for patients and represents our nursing community.

Donnell Hale

About Donnell

Donnell discovered NCODA in 2018 and instantly connected with their mission. She has cared for oncology patients since 1996 and feels honored to help these beloved individuals through their unexpected journey with cancer. She has worked for Texas Oncology for the past 17 years in several different roles in the out-patient setting. Her latest project has been assisting in the development of a new Virtual Care, Oral Anti-cancer Medication (OAM) Nurse Navigator program.

Amanda McCauley

About Amanda

Amanda is a fervent nurse and patient advocate. She has been an oncology nurse in Louisville, Kentucky for 17 years. Currently, Amanda serves as the assistant nurse manager for the system’s patient navigation team, cancer resource centers, and behavioral health programs. Her passion for oncology patients led her to join NCODA in 2018. Amanda is the Regional Leader for NCODA Region 5, has written articles for Oncolytics Today, served on the Fall Summit planning committee, and has assisted with developing a number of the resources produced by the NCODA Nursing Community.

Tonya Robinson

About Tonya

Tonya has spent her entire Oncology career at MD Anderson in Houston Texas which started in 2014. She tells everyone she believes oncology was her calling, where God has always planned for her to be. She has felt rewarded everyday to be a part of the patients journey. She was drawn instantly to NCODA and its mission. She is their biggest fan club member from Texas.

Debra Shapert

About Debra

Debra Shapert RN, MSN OCN is a Clinical Educator at Maryland Oncology Hematology. She has been working in various aspects of Oncology nursing for 30 years. Debra loves the concept of lifelong learning, and to support that in others. She is passionate about Oncology nursing and providing guidance to nurses at every level of their career.
