If you’ve read the first two articles in our series about our recent posts on the NCODA CoE MIP Accreditation program, you know it promotes a fully integrated team approach to patient care, leveraging the electronic health record (EHR) at every stage of the patient journey. This ensures total patient management, including clinical assessments, lab monitoring, adherence/toxicity tracking, financial assistance, patient education, Positive Quality Interventions (PQI), cost avoidance, waste reduction, and drug access.

Patients are also regularly monitored for adherence, side effects, and overall health outcomes which are key factors in successful treatment. Regular clinical assessments, including review of lab results and toxicity monitoring are also conducted to support timely interventions. If a patient shows signs of non-adherence, proactive strategies are employed to improve medication adherence, often before treatment even begins. Pharmacists collaborate closely with physicians, nurses, and other healthcare providers through the EHR sharing updates on patient progress and coordinating care, including medication adjustments.

Patient education is integral to total patient management. Medically integrated clinicians provide continuous education about medication changes, new prescriptions, and any changes in the patient’s condition. This empowers patients to stay informed and engaged in their care, promoting improved adherence and outcomes.
